ROLES OF SERVICE SYSTEM PARTICIPANTS

Individuals:
… determine life plans (to the greatest extent possible)
… are guiding force in the Service Coordination and Individual Plan (IP)/Individual Plan process
… participate in arranging for assessments
… participate in development of Individual Plan (IP) and in its implementation

Service Coordination:
… coordinates and participates in the development of the Individual Plan
… locates and assures access to support services
… facilitates communication among services
… involves consumer in life planning
… monitors individual plans
… facilitates the IP meeting
… documents the IP meeting and finalizes the IP based on this meeting
… assists individuals and families to identify current and potential natural supports
… assists individuals in developing community connections
… identifies gaps in assessment services and in the quality and quantity of other services and aggregates data relating to this
… ensures formal annual reviews/revisions of IP and more frequently, if needed
… cooperates with program providers to achieve these ends
… ensures monitoring based on the schedule outlined in the IP
… provides community education

Vocational and Other Support Providers:
… participate in the development of the Individual Plan
… document goals that provide teaching and implementation strategies as determined at the IP meeting
… provide teaching in economic self-sufficiency, self-direction, community, vocational, educational, social, language, motor, health, and self-care skills
… provide daytime support, direction, and instruction
… cooperate with the Service Coordinator and others in monitoring, evaluation, and service negotiation related to the IP

Residential Providers:
… participate in the development of the Individual Plan
… document the teaching goals and implementation strategies as determined at the IP meeting
… provide teaching in self-care, financial management, health, motor, language, social, leisure, environmental, and self-direct skills
… transportation to ongoing support services
… transportation to emergency services
… provide evening, night, weekend, and day support, direction, and instruction as necessary
… cooperate with the Service Coordinator and others in monitoring, evaluation, and service negotiation related to the IP

Developmental Disabilities Administration:
… provides funding
… designates and approves applicants to receive service coordination
… monitors provider agencies
… prescribes or ensures uniform expectations of Service Coordination and providers in the content, process, and format of IP
… develops the service delivery system capabilities necessary to assure that crises and emergencies are handled promptly and effectively
… assists in transfer of individuals among placements
… utilizes information regarding service system gaps for future planning
