… monitors individual plans
… facilitates the IP meeting
… documents the IP meeting and finalizes the IP based on this meeting
… assists individuals and families to identify current and potential natural supports
… assists individuals in developing community connections
… identifies gaps in assessment services and in the quality and quantity of other services and aggregates data relating to this
… ensures formal annual reviews/revisions of IP and more frequently, if needed
… cooperates with program providers to achieve these ends
… ensures monitoring based on the schedule outlined in the IP
